Engage SNP Members and
Drive Quality Despite Barriers



Many Special Needs Plan (SNP) members face unique circumstances that can prevent them from seeking or receiving the care they need. Many of those barriers are caused by Maslow’s Hierarch of Needs or Social Determinants of Health (SDoH). Regardless of the barrier, NCQA, CMS, and State Health agencies don’t make significant adjustments for SNP performance, so SNP plans have to find ways to engage these populations to drive results.
